WHS REPORTING POLICY

This policy outlines EK Recruitment (EKR) commitment to efficient identification of potential hazards and reporting and investigation of incidents with the purpose of encouraging early reporting and corrective action.

This policy applies to all EKR workers and worksites. A worker is any person who carries out work for EKF and includes employees, contractors and subcontractors, employees of contractors or sub-contractors, employees of labour hire companies, students gaining work experience and volunteers.

Roles and Responsibilities

The Executive Leadership Team (Director, general manager and supervisors)

Communicating to all employees this policy and the requirement to report hazards and incidents.

Manage all workplace hazards and incident by assessing risks associated with all reported hazards and incidents and implementing appropriate controls to address these and prevent reoccurrence.

Initiating an incident investigation using the incident report form as required.

Complying with the requirements for reporting notifiable incidents to WorkSafe.

Discussing and distributing incident investigation finding and preventative control measures to employees.

Updating the WHS Risk register to reflect changes to hazards, risk ratings and risk controls.

Workers

A worker Is responsible for:

Reporting all workplace hazards, incidents and near misses with 24hours.

Co-operating with incident investigations.

Implementing risk controls and reporting back on the suitability of these in reducing risk.

Following safe work procedures and instructions.

Hazard Management

Hazard Reporting

When a hazard is identified by a worker they are required to notify their supervisor on-duty who is to advise the office Manager. The office manager is to record the hazard on the database and ensure it is actioned.

Risk Assessment

The supervisor and/or general manager are required to consult with each other and relevant workers and complete a hazard assessment to determine the risk associated with the hazard.

Hazard Control

The supervisor and/or general manager in consultation with each other and relevant workers are required to develop a control strategy to control the hazard. A copy is to be provided to the office manager. The office manager is to upload a copy onto the database.

Review of Controls

The supervisor and/or general manager in consultation with each other and relevant workers are responsible for verifying the effectiveness of the implemented risk controls.

Incident Management

Incident Reporting

The supervisor and/or general manager must ensure that all incident and injuries that occur must be reported. All injuries that occur must be reported immediately to the supervisor and/or general manager. An incident report and investigation must be attended within 24 hours of the incident.

Notifiable Incidents

The supervisor and/or general manager must notify Worksafe of any notifiable incidents within 48hours of an incident occurring.

Site Preservation

The site of the notifiable incident must not be disturbed until WorkSafe inspector arrives or directs otherwise at the time of notification, unless there is a need to protect the health and safety of a person, aid an injured person involved on an incident or take essential action to make the site safe or to prevent any reoccurrence of an incident.

Incident Investigation

The supervisor and/or general manager must commence an investigation within 24 hours of the incident, completing an incident report with the purpose of gathering all the facts, identifying the hazards and causation factors followed by determining corrective actions.

Review of Controls

The supervisor and/or general manager in consultation with each other and relevant workers are responsible for verifying the effectiveness of the implemented risk controls.

Monitoring and Review

The Executive Leadership Team through line management will monitor and review the activities and undertakings within their area of responsibility to ensure compliance with this policy. The effectiveness of this policy will be monitored through the review of safety incidents which are reported, the effectiveness of risk controls and through audits conducted.
